If this is an emergency and an individual is in immediate danger, please call 911.
If you have witnessed an incident involving an individual with a developmental disability, please FILL OUT THE FORM BELOW or CALL US IMMEDIATELY at 513 732 4828 (During business hours) or 513-319-0179 (After hours).
Actual or suspected incidents that adversely affect the health and welfare of an individual with a developmental disability can be reported 24 hours a day, 7 days a week.
Please complete as much of the form below as possible and provide as many details as you can. We will do our best to maintain your anonymity so please consider providing at least a phone number and/or email so we can follow up with additional questions or clarifications. This will help to ensure that we are able to completely follow up with your reported issues/concerns.
Please note that this form does not take the place of the incident report for providers. Providers must follow all requirements outlined in OAC 5123-17-02 and Clermont County DD reporting procedures. Category A Incidents
Accidental or suspicious death, exploitation, failure to report, misappropriation, neglect, physical abuse, prohibited sexual relations, rights code violations causing risk, sexual abuse, or verbal abuse
Required Steps:
Ensure health and welfare by removing or minimizing threats and providing care. Notify law enforcement or Children’s Services immediately for criminal acts or child abuse.

Notify SSA, Investigations Department, or the On Call SSA Emergency number after hours. Emergency number: (513) 319-0179
Must occur within 0–4 hours and you must speak with a person. Notify the guardian and provider as soon as possible on the same day and document notification time. Submit a written report to Investigations by 12:00 noon the next working day. Fax: (513) 732-5020

This step is required even if verbal notification was made.
Non-accidental or non-suspicious death, attempted suicide, medical emergency, missing individual, peer-to-peer act, significant injury, law enforcement involvement, unanticipated hospitalization, or unapproved behavioral support
