If you have witnessed an incident involving an individual with a developmental disability, please FILL OUT THE FORM BELOW or CALL US IMMEDIATELY at 519-732-7000 (During business hours) or 513 319-0179 (After hours).
Actual or suspected incidents that adversely affect the health and welfare of an individual with a developmental disability can be reported 24 hours a day, 7 days a week.
Please complete as much of the form below as possible and provide as many details as you can. We will do our best to maintain your anonymity so please consider providing at least a phone number and/or email so we can follow up with additional questions or clarifications. This will help to ensure that we are able to completely follow up with your reported issues/concerns.
Please note that this form does not take the place of the incident report for providers. Providers must follow all requirements outlined in OAC 5123-17-02 and Clermont County DD reporting procedures.
